Preservation Rhinoplasty Surgery Technique by Dr Jay Calvert

Dr. Jay Calvert, President of the Rhinoplasty Society, says the preservation rhinoplasty technique has caught the interest of the Rhinoplasty community around the world. This philosophical approach to rhinoplasty allows the surgeon to attempt to preserve as much natural structure as possible. This includes the soft tissue ligaments and cartilaginous structures of the bridge of the nose. Dr. Calvert preforms over 200 rhinoplasty operations a year from offices in Beverly Hills and Newport Beach, California and has seen the interest in Preservation Rhinoplasty increase. His patients continue to become more selective when choosing a premiere plastic surgeon.  They want a plastic surgeon who can deliver outstanding results. This technique has become a topic of discussion.  In a blog post on his website, Dr Calvert discusses this preservation rhinoplasty in greater detail.  While Dr. Calvert feels that this technique may be useful in some very select cases, he cautions rhinoplasty surgeons to be careful when adopting this novel approach to an operation they already perform in a tried and true way.

"I am cautiously excited about preservation rhinoplasty.  And, I will evaluating this technique like any other new technique...with a keen eye on patient safety, reliable and reproducible results, and its ability to improve my patient's results," said Dr. Jay Calvert. "There is no substitute for thorough evaluation and study of techniques and the outcomes from implementing those techniques."

Dr. Jay Calvert, was installed as President of The Rhinoplasty Society annual meeting on May 16, 2019, in New Orleans, Louisiana. As President, he will be responsible for overseeing the operations of the Society, appointing committee members, and continuing to generate the highest quality rhinoplasty education in the world.  On October 9 - 11, 2020, The Rhinoplasty Society will partner with the Rhinoplasty Society of Europe to hold the 3rd International Meeting of the Rhinoplasty Societies in Berlin, Germany.  Dr Calvert will be lending his expertise to this meeting, as well.

Dr Jay Calvert will be speaking at the 15th annual Vegas Cosmetic Surgery meeting June 5-8, 2019 in Las Vegas. His lectures will include "In Situ Total Septal Reconstruction" and a Masters' Seminar on "Nasal Tip Architecture."

Innovative solutions MTF Biologics will highlight at The Aesthetic Meeting, include:

Renuva®: An allograft adipose matrix that conveniently treats small volume soft tissue defects in the face, breast and body. An alternative to fat grafting, Renuva is made from donated human adipose, or fat. It is produced from the highest quality tissue and gives patients the same benefits as fat grafting without the need for liposuction. A natural injection that does not require general anesthesia, Renuva can be administered in a doctor’s office.

Renuva will be presented by Dr. Haideh Hirmand at Hot Topics on Friday, May 17 at 1 p.m., and by Dr. Barbara Hayden on Sunday, May 19 at 10:30 a.m.

Meso BioMatrix®: A strong, thin xenograft tissue form for plastic and reconstructive surgery that offers surgeons a new option for patients requiring soft tissue repair and/or reinforcement such as suture line reinforcement and muscle flap reinforcement.
Profile®: A pre-shaped, costal cartilage allograft, Profile is used in rhinoplasty procedures. It allows surgeons to avoid having to perform additional surgery to obtain cartilage from a patient’s ear or rib, leading to less post-operative pain and scarring. Because Profile is made from human tissue, it is natural to the patient’s own body, which means fewer chances of infection or extrusion than synthetic implants.

Profile Costal Cartilage, Renuva and Meso Biomatrix will be featured in the Modern Techniques in Rhinoplasty Cadaver Workshop on Friday, May 17 at 2 p.m.

LipoGrafter®: LipoGrafter is a practical fat grafting solution designed for better graft survival. The easy-to-use technology offers surgeons a safe and efficient way to harvest a patient’s fat and deliver it in areas where the patient may require or seek additional volume.

To learn more, visit the MTF Biologics booth #845 at The Aesthetic Meeting 2019.

Dr Jay Calvert

Dr Jay Calvert Publishes On Rib Cartilage Graft

BEVERLY HILLS, Calif., May 12, 2014 – Dr Jay Calvert‘s publication in the May issue of Plastic and Reconstructive Surgery entitled Reconstructive Rhinoplasty: Operative Revision of Patients with Previous Autologous Costal Cartilage Grafts1 shows how a sample of revision rhinoplasty patients who have had at least two previous rhinoplasty operations including one with rib cartilage has the possibility of nasal repair when skilled rhinoplasty surgeons use the most current techniques.  Dr Jay Calvert, along with his co-author, Dr. Rollin Daniel are known for performing some of the most difficult secondary rhinoplasty operations in the world of plastic surgery. They and fellow author Dr. Anita Patel, discuss the high level of difficulty and the high revision rate associated with trying to repair patients’ noses when they have already undergone at least one operation with a rib cartilage graft.

Previously, patients who had been treated with rib cartilage grafts for secondary rhinoplasty were considered to be at the end of the road for surgical treatment.  However, following rib graft surgery, not all of these procedures turned out to be satisfactory to the patients and they sought out further surgery to try to improve both functionality and appearance of their noses.

“When I was approached by a group of patients with problems after rib grafts, I had to set their expectations and my own expectations appropriately.  There was no way to know what could be done in these difficult situations and we all had to work together to achieve the best results possible.”
- Dr. Jay Calvert

These operations were planned carefully and performed at the highest level possible.  The patients were followed closely to make sure that all that could be done was done to achieve a favorable outcome.  Most patients were treated with some form of re-grafting, either with more rib cartilage, ear cartilage or with Diced Cartilage and Deep Temporalis Fascia, a technique first reported by Dr. Rollin Daniel and Dr. Jay Calvert in the same journal in 20042.

The results were interesting in that the rate of revision surgery after the initial revision surgery by Dr. Jay Calvert was 24%.  The national average for revision among all rhinoplasty patients (primary or secondary) is 15%.  However, given the difficult nature of these patients’ problems, that was not unpredictable.  All of the patients had had at least two operations and some had had more than five operations.  The good news was that after those 24% of patients had their second operations with Dr. Jay Calvert, their results were improved such that they did not require any further surgery.
